GENERAL INSTRUCTIONS
To open a form in your browser…
- Click the download button for the form you wish to display
- The form will open in a new tab in your browser so you can easily get back to our website.
To save a form to your computer…
- Open it as instructed above.
- RIGHT click anywhere on the form.
- Select the “Save as” option on the menu that appears.
- Depending on your browser settings, you will either have the opportunity to choose where to save the file, or it may be automatically downloaded to a pre-determined location on your computer (usually to your downloads folder or to your desktop).
- You can then locate the form on your computer to work with as needed.
PRINTING
If you wish to print a blank form to fill in by hand…
- Open the form in your browser by following the general instructions above.
- You can then RIGHT click anywhere on the form and select “Print” from the menu that appears.
If you wish to print a filled form…
- Open the form in your browser by following the general instructions above.
- Fill in the form fields in your browser.
- You can then RIGHT click anywhere on the form and select “Print” from the menu that appears.
FILLING and EMAILING
In order to save a filled-in form to email to Monique…
- ***Important!*** You must first SAVE the form to your computer BEFORE filling it in.
- Once you have saved the form to your computer, you can locate the file, open it and fill it in.
- You will now be able to save the completed form back to your computer.
- To email a completed form to Monique, simply locate the form you filled in and saved to your computer and attach it to an email. There are several ways to do this depending on your operating system and your email client.
